Because safety is extremely important to us, volunteers may be asked to complete one of these screening forms after identifying the ministry area of interest and speaking with a staff member in that ministry. Please print and complete the appropriate form and FAX it to Elena Dunegan at (405) 302.1265 in the Volunteer Ministry office, drop it off at the church office, or mail it to: Crossings Community Church, ATTN: Elena Dunegan, Volunteer Ministries, 14600 N. Portland Ave., Oklahoma City, OK 73134.
